Volunteers

Volunteering with the Westview Gold Music Boosters is a wonderful way to participate and contribute to your studentʼs high school experience. Your participation is crucial to the success of this program and introduces you to a great group of parents. Your help is welcome any time. However, if you would like to volunteer to be a Chaperone, or help with Uniforms or with equipment as a Band Dad, you will need to be cleared through the Poway Unified School District by completing and submitting the following three documents:

1. Meganʼs Law Background Check, (Level 1 Volunteer Application): This form only has to be submitted once. Once you are cleared, you will not have to submit this form again during your entire tenure with PUSD.
2. Code of Conduct: Sign, date, and submit. This form only needs to be submitted once. However, if you change your clearance status from a Level 1 to a Level 2 (fingerprinting required), you will need to submit a new Code of Conduct along with your
   new clearance status.
3. Negative TB Test or X-ray: We must have a current Negative TB test or X-ray on file. TB tests are good for 4 years.

TB Testing Options:

1. Through your primary care physician.
2. Free through PUSD. In order to test through PUSD, you must first go into the Westview High School office (during their open hours) to pick up a Volunteer Verification Form. You will take this form with you to one of the PUSD designated test sites. Click here for the schedule of PUSD TB Skin Test Clinics.

Additional Paperwork that may be required:

Volunteer Site Transfer Request: This form is required to transfer your Meganʼs Law clearance status from one PUSD school to another PUSD school as you move or your child(ren) promote from Elementary to Middle to High School. If you were cleared through your childʼs Middle School, you must now transfer your clearance to Westview High School.

Tuberculosis Clearance Questionnaire: Please complete and submit this form if the x-ray results you submitted to PUSD is more than 4-years old.
